i would appreciate some information please. i am trying to complete an essay on pharmotherapeutics incorporating pharmodymanics and pharmokinetics regarding two drugs morphine and zofran. this is at level 3 nursing degree and is 2500 words.  i have attached the guidelines can you please reply, also do you use referencing within the last 5 years.

The INTRODUCTION should clearly identify:

-the assignment objective/aim

Care study context:

-this is the point where the relevant detail relating to the patient should be discussed within the context of pathobiology – be precise and stay focused!

-the rationale for the drug initiation needs to be clearly discussed at this stage and within the context of all other related therapeutic interventions;

-you need to demonstrate that you understand how the disease process(s) (pathology) is compromising normal body structure and/or function (human biology) and the proposed impact of therapeutic intervention with drugs (pharmacotherapeutics)

-the two drugs chosen should then be discussed either separately or together. Either way you need to be sure that the related pharmacokinetic and pharmacodynamic concepts which you have chosen to develop are clearly discussed in your own language and with appropriate literature support. You need to demonstrate what you understand and not what textbooks say!!

-within the area of pharmacokinetics your chosen concepts for discussion need to be within the following areas- administration, absorption, metabolism, distribution, and elimination

-in relation to pharmacodynamics you must:

a. identify the location of the human body molecule (or microbial target molecules) which the drug molecules are designed to target

b. in your own terms (along with literature support) explain the normal physiological role of these target molecules and how they are normally activated 

c. using your own terms, discuss how the drug molecules interact with the target molecules and the changes which this interaction is designed to bring 

d. identify as many areas in the body with similar target molecules and discuss the significance of this

the case study I was going to discuss was a fit and healthy 42 year old lady being admitted for an elective abdominal hysterectomy for menarrohagia and the medication I was going to discuss are patient controlled analgesia (PCA)  the drugs are MORPHINE AND ondansetron. Will also need to discuss any guidelines which may be applicable to the use of PCA

I understand we need to briefly mention/ discuss any other medications which would be used also.  Polypharmacy For induction of a general anaesthetic drugs of choice would be Propfol, suxamethanone, sometimes use sevoflurane gas, fentanyl and kytril I think these need to be mentioned in the case study and then state however the assignment is going to discuss the morphine and ondansetron and use the guidelines as above please.

The goal of this assignment is to review the results and implications of a recently published scientific article (Terry Hunt’s article in the course reader). Such reviews are often published by scientific journals as a means of communicating science to the general public. For example, a leading journal called Nature publishes short reviews in their News and Views section. For this assignment, you will write a review using the style, format, and structure used in the News and Views section of Nature. To help you write your review, I have also included specific instructions on style, format and structure below. Style and Format of the Review

The style and format of a review are dictated by its purpose: to review the results and implications of a recent scientific article for the general public. Thus, the style of a review is factual (reporting on the results) and interpretive (discussing the validity and implications of the results), but the language you use should not be too technical. In other words, the review should be informative yet engaging, and you should express your opinion regarding the validity and meaning of the results. Luckily, you have already learned a lot about the topic (Easter Island), so you should be prepared to formulate and express your opinion. If you need more information to help you form your opinion, you can read the articles cited by Terry Hunt and Jared Diamond (Chapter 2).

 

You should format your review as follows:

·         12 point font

·         1.25 inch margins on the left and right side of the page (the default in Word)

·         1 inch margins at the top and bottom of the page (the default in Word)

·         Double spaced

·         2-3 pages in length

·         Include a title

·         Do not use headings (the headings used below are only meant to indicate the structure of the review)

Evaluation of the Review

The final draft will contribute 100 points to your course grade (about 10% of the total – see Grading Scheme).

Introduction

 

This course introduces students to managing people and organizations in the context of change. As such it deals directly with some of the central issues faced business and its key functions.  The course is based on two books I have just written and deals with some of the core problems faced by all organizations, including management  and leadership, how individuals relate to their organizations, organizational design, international management, corporate reputations and branding, knowledge management, technology and people, and managing change and culture change and strategic HRM.   The two key themes in the course are change and internationalisation: change because there is rarely anything which is “fixed” about organizational life and HRM; one way of looking at organizations is that they are always in a state of becoming; internationalisation because looking at organizations and theories from other countries helps you to understand your organizations in this country.  Thus we will also look at some cases from Europe, the US and Asia, a few of which I have written myself in the course of previous research.

The course has four main learning outcomes and by the end of it you will

  1. Understand the key issues in organizational change and the management of change from the perspective of senior and middle managers and employees

  2. Understand the role played by people management in the process of managing organizational change and be able to evaluate the strengths and weaknesses of various approaches and techniques.

Guidelines for the Report

 

The purpose this report is to create an opportunity to apply the ideas and concepts discussed in class and the readings in the analysis of a real organizational situation. The situation may be drawn from your own experience, or from some public event on which information is readily available in newspapers, reports, etc. Your choice of situation will be an important one and will call for considerable judgment and discretion on your part in deciding whether it is feasible to use it for the purposes of the case study.

 

Be sure to maintain a professional stance in relation to matters that are sensitive. Confidentiality and the general conduct of the project is entirely your responsibility, so proceed with caution and great care.

 

In essence, the report invites you to do the following:

 

(a)   Identify some HR/people management/organizational change situation which appears to be amenable to analysis using the course material: the situation must be sufficiently complex to generate enough material for the writing of a case which meets the specifications described below.

(b)        Consider the concepts and general ideas discussed in class, and see which help make sense of the situation being described.

(C)       Engage in research activity which allows you to test the relevance of the different through the accumulation of relevant information and data.

(d)        Write up the report in a way which relates evidence to theory, to provide an appropriate analysis and explanation of the situation described.

 

Successful organizational analysis and design rests in an ability to examine any given organizational/HR/change situation so that its fundamental characteristics are made clear. It is not simply a question of spotting problems and applying appropriate solutions. Rather, it hinges on questions such as,

 

What is going on in the situation that I am analyzing?

How can I account for its characteristics and the way they are changing?

How can I make sense of the situation and arrive at an interpretation which  allows me to say something concrete about it?

 

HR analysis and design involves a process of thinking about a situation, constructing and reconstructing it in different ways that seem consistent with its nature, so that one can say something that may provide a basis for intelligent action. If the analysis has been done well, then the course of action which is appropriate for managing or changing the situation will become apparent.

 

Formal Requirements

 

The aim of this report is to get you started on the case study at an early stage, and to get you into a frame of mind which is analytical in the sense that it is constantly attempting to create a diagnostic reading relating theory and practice, taking the ideas discussed in the class and applying tern to the situation being studied. This report should be no more than 15 double-spaced typed pages and include the following:

 

(a)                     A brief account of the situation being investigated, providing sufficient information for the reader to understand the nature of the organization and its context.

 

This section should include relevant background information as well as a clear statement of the focus of your case study. For example, it is appropriate to provide information on the age, size, and history; of the organization, the product or services it provides, and the general nature of its environment the purpose of this is to orient the reader, so that he or she can acquire an understanding of the industry or sector with which you are dealing and the general trends it is facing. it is a good idea to do this as soon as you have selected a situation for analysis, preferably after the first week of class.

 

It should also offer a clear, concise statement of the problems, issues, or situations you will be examining as a focus for your case study. This is a critical dimension of your paper that cannot be underemphasized. Recall that your objective in this course is always to demonstrate the relevance of sound theory to practical organizational issues. To assure that you case analyses is firmly rooted in a specific situation or series of events, it is essential for you to explicitly identify this at the onset of your analysis. This will enable your reader to distinguish the direction and purpose of your analysis. In addition, it will provide you with a useful point of reference for determining what information about your organization should be included in your report.

 

(b) A series of sections applying ideas discussed in class to the case situation. Each class will give you some ideas through which you can see and understand the organization you are studying. It is your job to try and apply these ideas in practice, identifying the detailed ways in which the different theories/concepts relate to your particular organization.

1. Introduction:       Matched filters are robust

 

 

This paper shall experiment on the use of frequency-selective filters, matched filters, and Fast Fourier Transforms, and investigate the effect of bandlimiting spread-spectrum input signal on matched filter performance. The degree of bandlimitation to be examined is too varied between rather slight and very severe.

 

Ordinary signal waveforms for use in systems that do not use the spread-spectrum technique are usually quite sensitive to frequency-selective filtering: too much bandlimitation will usually cause severe waveform distortion, and the system performance will deteriorate rapidly as the filter bandwidth is reduced beyond what was originally specified for normal operation.

 

This is not the case with matched filtering. Matched filters are capable of tolerating many types of signal waveform distortion [ 1999], and perhaps their most fundamental property is that they are still able to detect the signal they were designed to match, even when the signal is invisible, immersed in noise, with a Signal-to-Noise Ratio (SNR) less than 0 dB. Other examples are well-known: the matched filter is still able to operate, with surprisingly small loss of performance when the input signal is coarsely quantised; or, in the case of a binary pseudonoise waveform, when a significant proportion of the signal “chips” are wrong [1965], where biphase-modulated signals are analyzed.  Yet another example of the various distortions that a matched filter is able tolerate is that of frequency-selective filtering of the received waveform. Even when a significant proportion of the frequency spectrum of the input signal has been removed, the matched filter still manages to detect the filtered waveform when it is immersed in noise 2006].

 

The paper shall examine quantitatively the way in which various measures of matched filter performance are degraded as the bandwidth of the channel carrying its input signal is decreased.

2. Matched filter performance measures

 

The measures of matched filter performance that we will be concerned with are:

  • Peak-to-Sidelobe Ratio (PSR), and
  • Signal-to-Noise Gain (SNG) (also called Processing Gain (PG))

 

The definition of PSR is the easier to understand. Ignoring noise, the output of the matched filter is the autocorrelation, of the bipolar () spread-spectrum signal sequence, consisting of M chips. This will be a sequence of length , symmetric about its midpoint M, where the peak value,  occurs. The sidelobe level is defined as the maximum of the absolute values of the sidelobes:  Straightforwardly, the definition of PSR is:

The definition of Signal-to-Noise Gain is more complicated:

where the Signal-to-Noise ratio, SNR, is defined, in the context of matched filtering, to be:

 

 

Assuming that the channel noise is additive Gaussian white noise (AWGN) with zero mean and RMS value  we have, at the input to the matched filter:

These two measures are both useful in characterising the performance of the matched filter: PSR is more to do with the detectability of the peak when the input signal is noisy: if S is large then the corresponding sidelobe is quite likely to be mistaken for the peak, and a false alarm will occur. By choosing the spread-spectrum waveform to be bipolar we have already ensured that P takes its maximum possible value, M, and the signal waveform design simply consists of making sure that S is reasonably small, so as to obtain a good (large) value of PSR. As the n-th sidelobe () consists of the sum of the sum of n fairly random  values, which by the Central Limit theorem, should be fairly Gaussian with mean zero (provided the spread-spectrum sequence itself is unbiased) and RMS value  Close-in to the main peak, then, it would not be too surprising to find a sidelobe of value around 2 or 3 times  This would imply that  would be in the range  but a clever waveform design might do better than this.

 

The SNG for the bipolar spread-spectrum waveform is easy to evaluate: the peak input signal level is obviously 1; the peak output signal is obviously M, and the square root in the expression for SNG is obviously  Consequently,

 

These values, , with  about 2 or 3, are the values corresponding to an unbiased bipolar spread-spectrum signal. But if that signal is subjected to lowpass filtering, the peak input signal will change, and so will the correlation peak, even though the matched filter’s bipolar coefficients are assumed to be unchanged. So filtering will affect SNG and PSR, and the question is: how much?

 

 

 

 

 

3. Task 1: Design of a spread-spectrum waveform

 

Here, MATLAB software will be used to design a spread-spectrum waveform.

What is required is a bipolar  waveform of M samples, which is unbiased. This means that M needs to be even, and the number of +1s needs to be equal to the number of –1s. This immediately disqualifies the popular binary m-sequences, for which M is confined to be, for some value of k. One way to find a suitable input signal is simply to generate sequences of M (pseudo)random bipolar numbers and reject those with too large an imbalance of +1s and –1s. This can be done easily, for instance, using MATLAB’s Gaussian random number generator randn.m, as follows: s = sign(randn(1,M));; or its uniform random number generator rand.m, as follows: s = sign(rand(1,M)-0.5);. Without too much searching, it is feasible to discover a sequence with zero imbalance, that is, zero bias. This is helpful in reducing the size of the correlation sidelobes.

 

A further design option is to oversample the signal, by a factor K. This can be done using the Kronecker product instruction in MATLAB, kron(.,.). Instead of increasing M, this is an alternative way of increasing the time-bandwidth product of the signal. It can be shown that the time-bandwidth product is equal to the Signal-to-Noise Gain, so this will have a direct impact on SNG, and PSR as well.

4. Task 2: Design of a family of signal postfilters

For this Assignment, it is required that you use MATLAB’s  fir1.m instruction to design the lowpass filter. This filter design instruction is parametrised by normalised cutoff frequency, and also the filter order, N. In addition, the type of window used in the design can be chosen from an extensive repertoire. Obviously, filtering the spread-spectrum waveform removes signal power, and apart from distorting the waveform, there is an overall reduction in the amplitude. To ensure a fair comparison, that is, to make the definitions of input SNR the same, it is necessary to rescale the waveform, increasing its amplitude so that the waveform maximum is unity, as in the original unfiltered case. This is easily done, by measuring the maximum value of the filtered waveform, and renormalizing the entire waveform by dividing by this value.

Calculate and plot the values of SNG in dB and PSR in dB for a range of filter normalised cutoff frequencies:  (The value of  corresponding to an allpass filter, is rejected by MATLAB).

 

The filter impulse response and magnitude of the frequency response should be plotted for the cases   Clearly, for  the values of SNG and PSR should be nearly those for the ideal case discussed in Section 3.

 

5. Task 3: Prefiltering the matched filter

Task 2 should convince you that matched filtering is robust to linear filtering of the spread-spectrum waveform. Now consider the idea that if this is so, then as far as the noiseless signal is concerned, it makes no difference whether the lowpass filter is placed on the far side of the channel in the transmitter following the waveform generator, or on the near side of the channel in the receiver, prior to the matched filter. This is just another way of saying that the PSR is the same in both cases. Formulate a formal proof of this assertion. In this alternative scenario, however, the noise also gets filtered by the lowpass filter, so that the SNR at the matched filter input will be better than in the case considered in Task 2. Modify the definition of SNG to model this situation, and repeat the SNG plot of Task 2 for this case.

The simulations thus performed, beyond mere laboratory demonstration, indicate the direction of further research and application of an alternative communication channel to possibly use lowpass instead of conventional bandpass filtering.  [1999] have similarly considered the robust detection of a spread-spectrum (ss) signal in the presence of unknown correlated ss interference and various pseudonoise additive. The general ss receiver structure they proposed is comprised by a vector of adaptive chip-based nonlinearities followed by an adaptive linear tap-weight filter, combining the relative merits of both nonlinear and linear signal processing [1999]. They further derived the optimum decision fusion filter for receivers that utilize hard-limiting (sign) chip nonlinearities. Data derived from their simulation and calculations demonstrate the performance of these schemes and offer comparisons with the conventional matched filter.

 

Similar experiments were performed by IBM for a white-Gaussian model in a similar context, though nonlinear effects of bandlimitation are not considered in the paper's analysis [].  The IBM research proves that for white noise, bandlimiting reduces output SNR by a few dB.

 

With no doubt, this area of spread-spectrum signalling is a rich area of future research and plethora of possible future applications

AQABA'S main port is to be relocated to the industrial zone in the southern port area, freeing up prime coastal real estate and allowing for much-needed expansion of port capacity. According to a master plan drawn up by the Aqaba Special Economic Zone Authority, the duty-free zone will become a regional hub for business and leisure, said Aqaba Development Corp (ADC) chairman & CEO . The enlarged southern port's capacity will grow to 100M tonnes a year as the result of two major expansion phases in the next 30 years, compared to the present maximum capacity of 30M tonnes/year. The current handling volume stands at 22M tonnes. "We started to worry about volume, and there is no room to expand within the present location,"  explained. "The other issue is that, with all the billions [of dollars] worth of urban tourism projects, ADC has attracted north of the main port area, we have a conflict of identity." Following best international practices, Jordan's only sea port will be broken up into separate business units to be offered for bidders, mainly on a joint-venture basis using the public private partnership model.

These units including the container terminal, a ferry terminal, marine services, a grain terminal and general cargo and ro-ro berths, in addition to an industrial terminal dealing mainly with phosphate, potash and fertiliser exports, which are Jordan's main mining exports.The ADC chief said general cargo, ro-ro, industrial and grain terminal might be offered as one package to interested operators. A decision about the oil terminal will be taken later this year,  added. Total investment for this plan is estimated at $1.5Bn and the project was originally scheduled to be concluded in 2020. However,  is optimistic that the work can be expedited and completed in 5-7 years.
